Margo (age 90) has been living alone in her apartment. Her family has contacted the county to request an LTCC because they feel she needs more help than they can provide.
A Long Term Care Consultant from Margo’s county of residence visits with Margo and her family to complete an LTCC. During the visit the consultant asks Margo and her family a series of questions about her daily living needs. Margo indicates she has trouble getting in and out of the bathtub, cleaning her apartment, making her own meals and doing her laundry. She also needs more daily social interaction than her family can provide. Margo has diabetes and has trouble taking a reading of her blood sugar levels to know the amount of insulin she needs.
The consultant discusses some of the community options available to Margo such as Meals on Wheels and chore services to help with bathing and laundry, etc. The consultant also indicates that there is a visiting nurse program available to help Margo with her medications. A community support plan is established and Margo receives help to access the service she needs.
