Prevents or delays nursing-certified boarding care facility placements by establishing whether the client meets an institutional level of care.

2. Can you name three people who might be part of the LTCC team?

Any one of the following may be part of the LTCC team:

Social Worker.
Public Health Nurse.
Family Members.
Friends.
Current Homecare Provider.

3. When is an LTC screening required for MA-LTC eligibility?

An LTC screening is always required in order to document that the client requires an institutional level of care when a person requests services through a waiver program or due to residence in an LTCF.

The LTC screening is in the form of a PAS for people residing in an LTCF, a full team DD screening for people requesting LTC services through the DD waiver, and an LTCC for people requesting LTC services through the EW, CAC, CADI or BI waivers. It is also required to receive services through the AC program.
