Minnesota adult MH-TCM services are defined as activities that are…designed to help adults with serious and persistent mental illness in gaining access to needed medical, social, educational, vocational and other necessary services as they relate to the recipient's mental health needs.

Case management services include developing a functional assessment, an individual community support plan, referring and assisting the person to obtain needed mental health and other services, ensuring coordination of services, and monitoring the delivery of services (Minnesota Statute, 245.462, Subd. 3).

The federal definition of targeted case management services is very similar.  Case management services means services with the purpose to “assist individuals eligible under the State (Minnesota) plan in gaining access to needed medical, social, educational, and other services.”

"Targeted” means that the State can plan (subject to federal approval) the case management services specifically for special groups/subpopulations of people who might benefit from the services.  Many States “target” case management services for adults with serious mental illness, children with emotional disturbances, people with developmental disabilities, or people with traumatic brain injuries.
