A Case manager talking to a client and taking notesIt is not uncommon under the heading of and other duties/responsibilities as assigned that a case manager could be involved in some part of a county’s MH-TCM intake process, but this is not a specific MH-TCM function of the case manager.  

However, it is the role of the case manager to assist the client in obtaining determination of ongoing eligibility for adult MH-TCM services. Minnesota statute requires that a diagnostic assessment be done to at least every three years to determine if the individual meets MH-TCM eligibility criteria related to SPMI. 

It is important that MH-TCM case managers be familiar with SPMI criteria. The case manager may be in the situation of having to explain this label to his/her clients or even mental health providers. Also, the case manager’s functional assessment might provide information to the diagnostician who is performing the diagnostic assessment for continuing eligibility for MH-TCM.
