The applicant must report all interest they or their spouse has in an annuity, including annuities they have funded or of which they are the beneficiary. Questions about interest in an annuity can be found on the application and request forms used for MA-LTC.

Who funded the annuity and with what type of funds were used to fund it, is not requested on any of the MA-LTC requests or applications. The DHS-5143, Required Annuity Information, is used to find out how an annuity on which the applicant or enrollee is named a payee was funded.

To Do: Look at your copy of the DHS-5143. Question 1 asks the person to indicate how the reported annuity was funded. This information is used to evaluate a transfer into an annuity to determine if it is uncompensated. More information about when to request this information will be discussed later in this course.

This information is not requested on any of the MA-LTC requests or applications. The DHS-5143, Required Annuity Information, is sent to an applicant or enrollee who reports them or their spouse has an interest in an annuity.

To Do: Look at your copy of the DHS-5143 again. Question 2 asks about annuity transactions. The applicant must complete, sign and return this form when requested to do so, in order to be eligible for MA-LTC.
