Requesting Verifications
Request verifications needed to determine eligibility. Do not require every applicant or enrollee who owns an annuity to verify every data element if the element is not necessary to determine eligibility. The data elements to request will become clearer as you learn more about what the data is used for.
Send an official request form, such as the Minnesota Health Care Programs Request for Information (DHS-3271) (PDF), to request verifications.Exception: A verification request form is not required when sending the Required Annuity Information (DHS-5143) (PDF) because due date information and the consequences of not returning the information can be found directly on the form.
