Electronic medical/health records are increasingly being adopted into behavioral health programs. Special care should be taken when using electronic records.

  1. Checklists and checkboxes need to allow for specific, individualized information/narrative about each client. This is especially true in the areas of symptom and functioning checklists.
  2. Service plan templates must allow individualization. This allows goals to be tailored to the client ’s recovery goals, and for objectives to reflect incremental progress.
  3. Drop-down menus mandate linkage between assessments and ICSP’s or between ICSP’s and contact notes are extremely helpful. 

Electronic files and signatures are being recognized as meeting documentation requirements.
