Step 4 – HealthQuest Submission
The fourth step, submitting a HealthQuest, is completed after the copy of the LTC insurance policy (or certificate of coverage) and the DHS-5426A are returned.
First, submit a HealthQuest, following your county's standard procedures, to request that DHS determine if the policy is a qualified Partnership policy. Include the information noted on your handout under sub-step 4a.
Provide the policy and the signed DHS-5426A to HealthQuest, noting the HealthQuest number on the cover sheet, if faxed.
Finally, be sure to enter a MAXIS case note about the submission with ***LTC Partnership*** as the title of the case note.
