Client Representatives

As you know, clients may choose an authorized representative

EPM Definition: A person authorized to act on a client's behalf for any of the Minnesota Health Care Programs (MHCP). An authorized representative may be designated by the client and may exercise all the rights and responsibilities of a client. (EPM 1.3.1.2)

to perform the duties required to establish and maintain eligibility. Clients may authorize a person to represent them in one of the following ways:

Completing the information on an application.
Providing a written request.
Providing a Power of Attorney.
Providing a letter of guardianship
Definition: A person appointed by the court to provide for the care, comfort and maintenance needs of a minor or incapacitated person who is known as the ward. A conservator takes precedence over a guardian as the authorized representative.

Providing a letter of conservatorship
Definition: A conservator is a person, official or institution appointed by the court to perform financial functions such as contracting, paying bills and investing assets for a minor or incapacitated person, known as the conservatee or protected person.

Note: Always enter authorized representative information in MAXIS whenever a client designates one. You may need to discuss who should receive system mailings, especially when the authorized representative resides with the client.
