Client Representatives
As you know, clients may choose an authorized representative
to perform the duties required to establish and maintain eligibility. Clients may authorize a person to represent them in one of the following ways:
- Completing the information on an application.
- Providing a written request.
- Providing a Power of Attorney.

Providing a letter of guardianship issued by the court.
