Communication with Designated Providers
Communicate with designated providers in the following manner:
- When the designated provider is the LTCF: Use the LTC/County Communication Form (DHS-3050) (PDF) to communicate the LTC spenddown amount initially and any time thereafter when the LTC spenddown amount changes. This will allow the LTCF to collect the correct amount.
- When the designated provider is providing LTC services through a waiver: MMIS sends a notice with the waiver obligation amount or spenddown amount to the providers on a monthly basis. You do not have to contact the provider with changes.
