What is the Elderly Waiver?

The Elderly Waiver program (EW) funds home and community-based services for:

People age 65 and older,
Who are eligible for Medical Assistance (MA), and
Require the level of care (LOC) provided in a skilled nursing facility (SNF), but
Who choose to reside in the community.

The EW program is administered by the Minnesota Department of Human Services and executed by county social services, public health departments, tribal agencies or health plans.

Referrals to EW Case Managers:

People interested in receiving services through the EW program, in addition to requesting MA-LTC services, must be referred to a case manager for a determination of the level of care.
For a person who is not currently enrolled in a Minnesota Health Care Program or is enrolled but is fee-for-service, refer to the client’s county social services department, public health departments, or tribal agency.

For a person who is enrolled in a Minnesota Health Care Program and is participating in managed care, refer to the client’s managed care plan.
