Level of Care Verification
In order to determine MA-LTC eligibility for a person residing in an LTCF, you must verify that an institutional level of care is required through a completed PAS or that the client is exempt from it.
This information is verified via the Physician Certification Form (DHS-1503) (PDF) and is completed by the placing health care professional and the LTCF staff. The LTCF must complete the DHS-1503 within 72 hours (excluding weekends and holidays) after an MA applicant or enrollee is admitted to the facility.
MA will not pay for nursing facility or certified boarding care services provided prior to the client's screening date.
