DHS-1503 Process
Long-term care facilities are responsible for completing and sending the DHS-1503 as soon as possible after an MA enrollee or a person applying for MA is admitted.
Upon receiving the DHS-1503, enter the date received and the name of your county in the Local County Agency Use Only section at the bottom of the form. After determining eligibility for MA-LTC, you will sign the form and enter the date you return the form to the LTCF.
To Do: Enter the date and agency on Emily’s form. You can't sign the form or return it until eligibility has been determined.
