It is imperative that prompt communication between you and the waiver case manager
The case manager may be a county or tribal public health nurse or social worker, or an MHCP managed care plan case coordinator. The case manager is responsible for ensuring that the client meets the criteria to receive services through the waiver program and coordinates the receipt of the needed services.
