DHS-5181 – Sections B and C
In Section B you see that Zach (the case manager) indicated that Padma:
- Was screened on the 15th of last month.
- Anticipates her EW services will begin as of the LTCC.
- Applied for MA.
Zach would also indicate on the form if Padma was an MA enrollee and if the appropriate form (DHS-3543) was being completed. He may also indicate if any changes take place once she is eligible for the waiver.
Zach was kind enough to let us know in Section C that Padma was admitted to the hospital on the 7th of last month and was discharged on the 29th.
