Procedures have been established for the use of the DHS-5181 for you and the case manager to follow.

Who? Case Manager.

When? Immediately.

Question 2.

When? Immediately.

Question 3.

When? Not required. The DHS-5181 is not required because Jake is not receiving services through a waiver program. You'll learn more about another form you may need to complete to notify the LTCF later in this course.

Question 4.

When? Within 10 days. The worker must return the DHS-5181 no later than January 20 to report that eligibility is pending for income verification. If the verification has not been returned by February 14 (45 days from the time the DHS-5181 was initially sent by the waiver case manager) the worker must send another DHS-5181 indicating eligibility will soon be denied because income verification has not been provided.
