To Do: Take out Padma’s DHS-5181 found in Padma’s training case file.
Notice that Padma’s case manager, Zach Benson, has completed Section A with his contact information and the worker’s contact information. He has also provided Padma’s name, date of birth and the waiver program.
Zach also indicates that a conversion to the EW program will occur. The case manager chooses whether entry into the waiver program is considered a diversion or a conversion. This information is important in order to code the MAXIS STAT/DISA panel correctly.
