DHS-5181 – Section A

To Do: Take out Padma’s DHS-5181 found in Padma’s training case file.

Notice that Padma’s case manager, Zach Benson, has completed Section A with his contact information and the worker’s contact information. He has also provided Padma’s name, date of birth and the waiver program.

Zach also indicates that a conversion to the EW program will occur.  The case manager chooses whether entry into the waiver program is considered a diversion
Diversion means that the client is requesting or enrolled in the waiver program and has avoided a placement in an LTCF.
or a conversion.
Conversion means that the client was residing in an LTCF and is now being converted to a waiver program.
This information is important in order to code the MAXIS STAT/DISA panel correctly.
