Designation Process - Step 1

The first step in designating DHS as the Preferred Remainder Beneficiary (PRB) is to complete and send the Annuity Beneficiary Designation Form (DHS-5036) to the client.

Remember! You must send a separate form for each annuity that meets the requirement to designate DHS as the preferred remainder beneficiary.
