Purpose of the Annuity Beneficiary Designation Form
The purpose of the Annuity Beneficiary Designation Form, whether the DHS-5036, is to request the annuity issuer to designate DHS as the PRB of the annuity indicated. This purpose is listed at the top of the form’s first page.
By signing the form, under the statement found in the middle of the first page, the owner(s) of the annuity is requesting an update to the annuity’s beneficiary designation. Each form explains why the request is being made and what it means.
An authorization of release is found on the second page. By signing this authorization the annuity owner(s) agree to allow DHS to release the information listed on the form to the annuity issuer.
