Designation Process - Step 2

Let’s look at Step 2 found on the PRB Designation Process (PDF) handouts. This step begins when the DHS-5036 is returned, or when the DAIL/WRIT message you set appears on your DAIL report.

Form Not Returned:

If the form has not been returned the applicant is not cooperating with this annuity eligibility requirement.
  • Calculate a period of ineligibility for MA-LTC.
  • Deny or close MA-LTC eligibility by completing and sending the Notice of Action for Payment of LTC Services (DHS-4915).
  • Add a worker comment to the MAXIS denial or closing notice indicating an additional denial or closure notice is being sent.
  • Update the MMIS RLVA screen with the period of ineligibility. The handout, RLVA Screen – Annuities (PDF), provides you with information regarding this coding.
  • Send the Lead Agency Case Manager/Worker Communication Form (DHS-5181) or the LTC/County Communication Form (DHS-3050).
