Step 3
Continue to Step 3, found on the PRB Designation Process (PDF) handouts when the DHS-5036 is returned by the applicant, and all signatures and information have been provided. Step 3 has been broken down into sub-steps below.
a. First, make a copy of the DHS-5036 for the case file.
b. Then complete the Issuer of Annuity Notice of Obligation (DHS-5037).
To Do: Take out your copy of the DHS-5037 and take a guided tour of the DHS-5037.c. Make a copy of the completed DHS-5037 for the case file.
d. Send the original of the completed DHS-5037 with the signed original of the DHS-5036 to the annuity issuer.
e. Set a MAXIS DAIL/WRIT for 30 days (the day the DHS-5037 should be returned.)
