Step 4
Let’s look at Step 4 found on the PRB Designation Process (PDF) handouts. When you receive the second DAIL/WRIT set for the return of the DHS-5037, check to see if the DHS-5037 has been returned.
- If the completed and signed DHS-5037 has been returned, continue to Step 5.
- If the DHS-5037 has not been returned, attempt to contact the annuity issuer by phone to request the completion and return of the form. Allow an additional 10 days for the issuer to return the form. Complete another MAXIS DAIL/WRIT to track for return.
If the issuer does not return the form after the additional 10 day time, you must send a copy of the signed DHS-5036 and a copy of the completed DHS-5037 to the DHS Special Recovery Unit (SRU). SRU contact information can be found on your handout.Do not deny or close MA-LTC for this requirement while the DHS Special Recovery Unit follows up with the annuity issuer. Consider this requirement met until you hear from SRU.
