A list of resources is provided on the handout MA-LTC Community Resources (PDF). When you have time, look at the resources on the handout to learn more about the community supports that are available for clients and their families.

DHS launched MnCHOICES in 2013. A person of any age with a disability or in need of long-term services and supports may use the MnCHOICES person-centered process to help make decisions about long-term services and support needs. MnCHOICES is an assessment and planning tool used by counties, tribal nations and managed-care organizations. This assessment is a free, in-person visit with a certified assessor. Clients and their caregivers work with the assessor to develop a community support plan and obtain services.
