Transitional Services

“Transitional Services” is a service available for people 18 years of age or older who are eligible for the CADI waiver. Funding for these services is available when there are no other funding sources available, including Emergency Assistance.

Transitional Services covers items, expenses and related supports for a person to move to their own home (a setting that a person owns, rents, or leases) from the following licensed settings: adult foster care, hospitals, intensive rehabilitation treatment and Rule 36 settings, ICF/DD facilities, and nursing facilities.

Transitional services are limited to $3,000 per transition period (or move). A transition period can occur once every three years.
