Monthly Medical Spenddown

As mentioned earlier in this section, you do not interrupt the certification period to change from an LTC income calculation to a community income calculation. When a client has a medical spenddown in the months of a certification period following the LTC income calculation, use a monthly medical spenddown. If the client cannot meet the monthly medical spenddown, provide a 10-day notice of closure.

Example:

Alfred is an MA-LTC enrollee. Alfred, who is single and certified disabled by the SSA, was admitted to an LTCF in January. His current certification period is May through October. He is discharged home on August 22. He will not be receiving LTC services in his home. He will have a monthly medical spenddown.

Action:

Use the LTC income calculation through July, the month prior to discharge, because Alfred does not have a community spouse. Determine eligibility for August using an MA Method ABD community income calculation. Because 10-day notice cannot be provided for August or September, the client will not have to meet the monthly medical spenddown until October.
