The information in the Caregiver tab is copied from two question groups in the Caregiver domain of the MnCHOICES Assessment.
As long as the Assessment has an open status, the Certified Assessor can log in to the Assessment and make edits to the information. Any edits made in the Assessment are automatically copied over to the Support Plan.
Because the Assessment has to be closed before a CSSP can be created, no changes to the information copied over from the Assessment can be made after the CSSP has been created.
The information in this section lists people who support the person and who do not get paid for the work they do (often referred to as informal caregivers).
Review how the needs of the caregiver were proposed to be met, then use the text box to document and update the information based on final decisions and choices of the person and the caregiver.
This section contains information that was entered in the Assessment based on what was learned from the unpaid caregiver during a caregiver interview.
Only questions and responses that were documented in the Assessment are carried over. If a question from the interview was not answered, it does not appear in the Support Plan.
Click the Add Caregiver button to add a new caregiver.
Enter the name of the caregiver in the Name column.
Use the dropdown menu in the Caregiver Relationship column to select the relationship of the caregiver to the person.
If the caregiver lives with the person, click the checkbox in the Lives with Person column.
Use the dropdown menu in the Caregiver Role column to select from the following choices:
Click one or more checkboxes to select the type of care the caregiver will be providing in the Type of Care column. You may select more than one checkbox.
To remove a caregiver, click the "X" button at the right of the row. All of the data in that row will be deleted.
Enter any additional notes about caregivers in this text field. This field prints in the CSP and CSSP.
