General information about this tab

The My Plans tab provides a quick way to find and open existing CSP, CSSP, Rate Plans and LTSS Evaluations.

Only documents created in the Support Plan—CSPs, CSSPs, Rate Plans and LTSS Evaluations—can be viewed on the My Plans tab.

If there are no documents which match the selected View, Status and Document Type options, “No data to display” will appear in the documents table.

View dropdown box

The View dropdown box contains the following options:

  • Assigned to me as Plan Owner
  • Assigned to me as Shared Owner
  • Assigned to me as Secondary Owner
  • Assigned to me as Plan Aide
  • My Agency
  • My Teams

Which documents display in the table below depend on which option is selected. For example, to view all documents that exist for your agency, select My Agency in the dropdown.

Assigned to me as Plan Owner is selected by default.

Change My Agency button

If you work for more than one agency, by default, you will see the documents for the agency that you entered when you logged in. To change which agency’s documents appear in the table below, first select My Agency in the View dropdown box. Next, click Change My Agency and select the agency whose documents you want to view.

Status

Choose which type of documents you want to view: Open, Complete or Closed. Only documents with the status you choose will be displayed in the table below.

The Open radio button is selected by default.

Document Type

Choose which type of document you would like to view: CSP, CSSP, Rate Plan or LTSS Evaluations. Selecting All will show all CSPs, CSSPs, Rate Plans and LTSS Evaluations.

It is not possible to view other types of documents—such as Assessments--on the My Plans page. To see whether the person has an Assessment, search for the person on the Search tab.

All is selected by default.

Show/Hide Columns

Click the Show/Hide Columns hyperlink to bring up a list of all the columns in the documents table. Uncheck a checkbox next to the name of a column to hide that column. Check the checkbox to show the column.

The ID column is the only column which cannot be hidden.

Your settings are saved when you navigate to another tab or log out. If you hide the Zip column and then log out, for example, the column remains hidden the next time you return to the My Plans tab.

Click the “x” in the upper right corner of the dialog box to close the dialog box. You can also click the Show/Hide Columns hyperlink to close the dialog box.

Documents table

Lists the documents which match the settings you selected for View, Status and Document Type.

Click any column header--“ID” for example-- to sort the table by that column. Click the header again to reverse the sort order.

You can pick up any column by the green column header cell and drag it to a new position in the table. The table will retain the new column order the next time you return to this screen. By using Show/Hide Columns and dragging/dropping columns, you are able to organize the data in the documents table to include only the information you want to see, and to display it in the order that you choose.

Filtering dating in the documents table

To limit which documents appear in the table, click the filter icon to the right of the text box and select the appropriate option:

  • Begins with
  • Contains
  • Doesn’t contain
  • Ends with
  • Equals
  • Doesn’t equal
  • Like (‘%’,‘_’)

Next, enter some text in the text field to the left of the filter icon for that column.

For example, to show all documents where the person’s last name ends with “son”, click the filter icon and select “Ends with”, then enter “son” in the text box to the left of the icon.

A new row appears below the table showing the filter which is being applied to the table.

You can apply more than one filter at a time. Repeat this procedure to filter based on the content of one of the other columns in the table.

To remove a filter, click the Clear hyperlink on the right side of the screen for that filter.

ID column

The identification number of the document. Click the document identification number to open this document.

Last column

The person’s last name.

First colum

The person’s first name.

Middle column

The person’s middle name.

Gender column

The person’s gender.

DOB column

The person’s date of birth, listed in the format mm/dd/yyyy.

SSN column

The person’s Social Security Number.

PMI column

The person’s eight-digit Person Master Index (PMI) number.

Type column

The type of the document. This will be one of the following:

  • CSP
  • CSSP
  • Rate Plan
  • Evaluation

It is not possible to see whether a Rate Plan was created at the person level or the document level.

Create Date column

The date when the document was created.

Status column

The status of the document. Only those documents which match the selected radio button for Status above will be shown. To see documents with a different status, change the radio button selection in the Status field.

Plan Owner column

The person responsible for completing the plan.

When a document is created, the person who creates the document becomes the plan owner.

The plan owner can be changed on the About Plan tab.

Delegate Agency

The agency that the document creator works for.

Lead Agency column

The lead agency responsible for the plan.

City column

The city in which the person lives.

Zip column

The zip code of the person’s residence.

Modified By column

The person who last modified this document.

This column is not being used at present.

Modified By Delegate/Lead Agency column

the delegate or lead agency who last modified this document.

This column is not being used at present.

Modified column

The date when the plan was last modified.

Search

To search for a word or phrase on the current page, hold down the Control key and press the F key (Ctrl + F). Enter your search text in the text field that appears at the top of the screen.

Browser settings

To increase or decrease the size of the print on the screen use your browser setting to change the zoom. To change your screen display, on the right hand side of the URL choose
  Internet Explorer and Edge: The magnifying glass with a plus sign will allow you to increase and decrease the “zoom” on your screen display.
  Chrome: The ellipsis will bring up a menu of options. Choose zoom to change your screen display.
