General information about this tab

The data displayed in the two tables on this tab is information about the person that is copied over from the Assessment.

Only information documented in the Assessment is displayed on this tab. For example, there are many options that can be checked in the Assessment related to preferences and support instructions. Only the options that are actually checked are brought over to the Support Plan.

Some information may display as blank lines in the Support Plan that was not entered in the Assessment.

Data in the two tables can’t be edited in the Support Plan. Any additions or updates to the data must be done in the Assessment.

Activities of Daily Living (ADLs) table

Items checked under Preferences or Support Instructions in the Assessment are copied from the Assessment to this table and listed separately with bullets. This includes any items under Preferences or Support Instructions that were checked as “other” and hand entered by the Assessor.

In addition, the decisions made about whether or not the person needs training or skill building to increase independence is carried over for each ADL.

Instrumental Activities of Daily Living (IADLs) table

Data from the IADLs domain of the Assessment is carried over into this table. The IADLs are listed in alphabetical order. Any items checked under preferences or Support Instructions are carried over to the Support Plan and listed separately with bullets. This includes any items under Preferences or Support Instructions that were checked as “other” and hand entered by the Assessor.

Decisions made about whether or not the person needs training or skill building to increase independence is carried over for each IADL.

Support Instructions for Supervision text field

This text field is used to provide specific instructions for providers about the person’s preferences and support instructions about what the person needs in relationship to cuing, oversight and monitoring. Sometimes referred to as the type and amount of supervision the person needs.

If you add or edit text in the this text field, click the Save button in the upper right of the navigation bar before logging out or navigating to a different tab in the Support Plan.

Text entered in this text box does not print in the CSP, CSSP, Rate Plan or Service Agreement Report.

Browser settings

To increase or decrease the size of the print on the screen use your browser setting to change the zoom. To change your screen display, on the right hand side of the URL choose
  Internet Explorer and Edge: The magnifying glass with a plus sign will allow you to increase and decrease the “zoom” on your screen display.
  Chrome: The ellipsis will bring up a menu of options. Choose zoom to change your screen display.
