Offline use

If you will not have internet access while conducting the evaluation, open the PDF version of form DHS-7611A from the DHS eDocs site. Click the Save a copy button and save the document on your computer. Use the saved version of the document during your meeting with the person, and copy the information to the Support Plan when you return to the office.

Save button

After entering or editing information on this page, click the Save button.

Close button

After you have finished entering data in the evaluation and are ready to submit the data, click the Close button. A dialog box will appear asking you to confirm that you want to close the evaluation. To close the evaluation, click "Yes". To continue working in the evaluation, click "No".

After the evaluation is closed, none of the data in the evaluation can be edited.

General evaluation notes

Radio buttons indicate you may choose only one answer.

Check boxes indicate you may choose multiple answers.

Some responses to questions will add additional questions to the evaluation.

Questions 6 & 8

If the response to the question is "Some of the time" or "Rarely", a follow-up question and text box will appear.

Question 12

If the answer is "a", three additional questions will appear. If the answer is "b", one additional question will appear.

Question 14

If you check the checkbox next to any of the services or supports, use the text field to give more information about the person's answer.

Question 15

If "yes" is selected, an additional question and text field will appear.

Browser settings

To increase or decrease the size of the print on the screen use your browser setting to change the zoom. To change your screen display, on the right hand side of the URL choose
  Internet Explorer and Edge: The magnifying glass with a plus sign will allow you to increase and decrease the “zoom” on your screen display.
  Chrome: The ellipsis will bring up a menu of options. Choose zoom to change your screen display.
