General notes about this tab

Because your training zone document has a mock PMI, any changes you make in the person information tab will result in the following error when you try to save:

This error only occurs in the training zone, not the live production environment of MnSP.

The information on this tab should be reviewed and edited as needed in the CSP and CSSP.

Person information is copied over from the Assessment application.

Providers tab: If you do not find the professional you are looking for in the drop down menu, choose Other and type their profession in the text field below the drop down menu. You can add the person’s name and other information in the Comments section.

Some information can be edited in the Support Plan, while other information must be edited in the Assessment. The first name, for example, can be edited in the Support Plan, while the county of financial responsibility must be edited in the Assessment.

What Sections can be edited and what updates the Assessment database?

  • Edit in MnSP and it updates the MnA 1.0: Name, Age, Gender, Maritial Status, Race, Address, Email, Phone, Financial Worker
  • Edit in MnA only: Veteran
  • Edit in MnSP, does not update assessment database: CFR, Emergency Contact, Health Insuarance & Payers, Providers and Decision Makers

If you update any person information in the Assessment, it will be automatically updated in the Support Plan the next time you log in to the Support Plan, or the next time you return to the Person Information tab.

Before closing the assessment and CSP, the Assessor should review the contents of this page with the Planner.

Do not enter hyphens when typing phone numbers or Social Security Numbers.

Once a Phone, Address, or Email is saved it cannot be deleted, only changed.
Workaround: Enter generic data in the field, such as '999-999-9999'.

Opening and closing sections

The Name section is open by default when you navigate to this tab.

You can open or close any section by clicking on the gray bar at the top of the section.

To open all sections at once, click the Expand All button. To close all sections, click the Collapse All button.

Saving

If you make changes on this page, click the Save button before logging out or navigating to a new tab.

Search

To search for a word or phrase on the current page, hold down the Control key and press the F key (Ctrl + F). Enter your search text in the text field that appears at the top of the screen.

Browser settings

To increase or decrease the size of the print on the screen use your browser setting to change the zoom. To change your screen display, on the right hand side of the URL choose
  Internet Explorer and Edge: The magnifying glass with a plus sign will allow you to increase and decrease the “zoom” on your screen display.
  Chrome: The ellipsis will bring up a menu of options. Choose zoom to change your screen display.
